Most students with an attempt made 1 of 3 mistakes: they thought the real exam would be easier than their practice, they took their NBMEs online instead of timed and offline, or they never got above 68% on NBME 31, 32 and 33 and booked off a trend of 65s with 1 high form. ## Page text USMLE Vault · Step 1 · Second attempt If you failed Step 1 once For IMGs with 1 attempt on their record. 5 of 6 of my students with a previous attempt passed. This is the order they went in. By Dr. Marish Asudani · Published 22 October 2026 What should I do if I failed Step 1 once? Stop and find out why first. They thought the real exam would be easier than their practice. It wasn't. They weren't practising properly, timed and offline. They gave their NBMEs online, so they never understood the timing of the real exam. The online version gives you 50 questions in 75 minutes. It feels comfortable, and it inflates your score by about 4 to 6%. Offline, you train the real pace: 20 questions in 30 minutes, block after block. That's why NBMEs are always taken offline. Why NBMEs are always offline → They didn't do enough NBMEs, and they never got above 68% on NBME 31, 32 and 33. So they believed what their friends told them. Maybe they were getting a trend of 65s, then 1 NBME came back high, and they booked the exam. That is wrong as well. Trend versus spike → 2 Be honest Which one were you? For anyone to give Step 1 confidently, you have to get more than 68% on NBME 31, 32 and 33 .
