Why First Aid is not enough on its own

"Just read First Aid." The most repeated advice in Step 1 prep. Here's what it leaves out, and what fills the gap.

By Dr. Marish Asudani Β· Published

Why is First Aid not enough on its own for Step 1?

First Aid on its own is a very dense book. It was written to remind, not to teach. You need something to decode it, then a way to apply it, then a way to recall it: decode with the First Aid Guide, apply with Mehlman and UWorld, recall with FA Foundation Anki. Then First Aid is enough.

Read this firstFirst Aid is the backbone of Step 1. I'm not telling you to drop it. I'm telling you why reading it alone doesn't work.

Every line in it assumes you already understand it. That's why IMGs spend 3 hours on 1 page, Google half of it, and still retain nothing.

Every page has to answer 5 questions: what it is, what is important, why it is important, how it is important, and how to understand it. The book prints none of the 5 answers.

Job 1Decode it: the 5 questions every page has to answer

⭐ THE MISSING PIECE

A page of First Aid gives you conclusions. To learn from it, you need 5 answers the book never prints. The First Aid Guide (what it is) puts 1 feature on every page for each of them:

You need to knowThe feature that answers it

1 real page, decoded: First Aid p. 596, kidney embryology (free on the Vault).

Focus On: "The embryology timeline itself is low-yield. Don't memorize weeks. Focus on understanding reciprocal induction between the ureteric bud and metanephric mesenchyme, and master Potter sequence completely."

Exam reality: "I didn't see any question from renal embryology during prep or on the real exam. Potter sequence is the only very important concept from embryology."

The Trap: the question may give an unusual cause of oligohydramnios (maternal ACE inhibitor use, bilateral renal agenesis, posterior urethral valves) and ask what happens to the fetus.

The book gives you the same page with none of those 3 sentences. The page opens by telling you not to study half of it.

Job 2Apply it

Understanding a page and answering a question on it are different skills. You apply First Aid in 3 layers, each harder than the last:

  1. Test Yourself MCQs, on that page. The same minute you read it. With a Rule out button that strikes an option through, so you practise elimination, not just recognition.
  2. Mehlman's videos for that system. Short videos that show you how the exam asks each page: which clue rules an answer in, which rules it out. How to use Mehlman β†’
  3. UWorld, system by system. Every miss traced back to its First Aid line. The RPSR method β†’

Retention doesn't happen while you read First Aid. It happens when you apply it.

Job 3Recall it

The last job is pulling every line back out of memory, without the page in front of you. That's the FA Foundation Anki (how to run it):

Hesitate on a card β†’ open the page, read the line and a couple above and below. That's First Aid, read again, but actively.

A FA Foundation Anki card, flipped: First Aid p. 597 on the card, and Study this in First Aid under the answer.
